We are looking for Full-Time Care Coordinators who are passionate about Chronic Care Management (CCM). This is a phone-based role designed for clinical professionals who thrive on conversation and organization. In this position, you are the consistent voice that helps patients navigate their health journeys from the comfort of their homes.

The Mission: Connection & Coordination

This is a productivity-driven role where your primary “tool” is your voice. You’ll spend the majority of your day on the phone, building relationships with patients and ensuring no one falls through the cracks of the healthcare system.

What Your Day Will Look Like:

  • Patient Advocacy: You’ll conduct monthly check-in calls, making between 30-50 outbound calls a day to ensure your assigned patients are meeting their health goals.
  • Education & Support: You’ll help patients understand their chronic conditions and empower them to stick to their care plans.
  • The “Glue” of Care: You’ll coordinate with pharmacies, provider offices, and community resources to make sure every part of the patient’s care team is on the same page.
  • Care Planning: You’ll identify gaps in care or social challenges (like transportation or food access) and update personalized care plans within the EMR.
  • Meticulous Documentation: You’ll accurately log every interaction, maintaining the high standards of HIPAA and patient confidentiality.

What You Bring to the Team

We’re looking for clinical pros who are comfortable with high call volumes and love the structure of a remote, metric-driven environment.

The Essentials:

  • Clinical Background: You have 1-3 years of experience as a CMA, RMA, LPN, or RN. (LPNs and RNs must have an active multistate license or state-specific certification).
  • Communication Skills: You are a clear, empathetic communicator who enjoys being on the phone for 7-8 hours a day.
  • Tech-Savvy: You’re experienced with EMR/EHR systems and can work independently without losing focus.
  • Reliability: You are organized, dependable, and ready to stick to a fixed 40-hour weekly schedule (Monday-Saturday, 8:00 AM – 6:00 PM in your supported timezone).

Bonus Points:

  • You have experience working with geriatric patients.
  • You’ve done care coordination or case management before.
  • You are bilingual (English/Spanish).
