RN Care Manager

 Lakeland Care has been a pillar of support for frail elders and adults with disabilities across Wisconsin. Our mission is simple: to help people live as independently as possible. We don’t just “manage” care; we empower individuals, working closely with their families and providers to create high-quality, cost-effective care plans that actually work.

We are looking for a Registered Nurse (RN) Care Manager to support our members in Manitowoc and Calumet counties. This is a “home-based” role with field travel, meaning you get the flexibility of working from your home office while spending your days out in the community, seeing the real-world impact of your work.

What You’ll Be Doing Every Day:

• Member-Centered Advocacy: You’ll be the lead coordinator for your members, getting to know their unique situations and tailoring long-term support to fit their lives.
• Clinical Expertise: You’ll use your nursing background to assess needs, identify health goals, and ensure every member is receiving the right level of care.
• Collaborative Planning: You won’t work in a vacuum. You’ll actively engage with providers and care teams to foster a sense of community and achieve the best possible outcomes.
• Empowerment: Your goal is to promote well-being and independence, helping our members stay in their homes and communities for as long as possible.
